MRI EXAMINATION OF THE RIGHT KNEE WITHOUT CONTRAST

INDICATION FOR STUDY:         717.9.  Pain.

FINDINGS:               Axial, sagittal, and coronal images were obtained on the 1.5 Tesla GE MRI.  There is a Baker’s cyst.  There are significant degenerative changes and degenerative joint disease at the medial aspect.  There is significant joint space loss.  There are degenerative cystic changes and possibly osteonecrosis especially at the tibial aspect, but also noted at the femoral condyle.  There is degenerative meniscal tear along the medial aspect.  This is noted in the posterior horn of the medial meniscus.  There are intrasubstance degenerative changes in the remainder of the menisci.  The anterior and posterior cruciate ligament, medial and lateral collateral ligament, and quadriceps patellar tendon are intact.  There is small amount of joint fluid.

IMPRESSION:         Significant degenerative changes and degenerative joint disease.  Significant changes noted on the medial aspect both at the femoral condyle and medial tibia, especially in the tibial region.  These appear chronic in nature.  There is a degenerative meniscal tear in the posterior horn of the medial meniscus.  There is small Baker’s cyst.  Would recommend further evaluation as needed.

MRI EXAMINATION OF THE RIGHT KNEE WITH AND WITHOUT CONTRAST

INDICATION FOR STUDY:           Osteochondromas.  Pain.  Prior x-ray evaluation from XXXXXXX.  Degenerative joint disease.  844.9.

FINDINGS:               There are changes consistent with osteochondroma, especially noted off the posterior aspect of the tibia.  This area does not appear to show significant abnormal enhancement.  It is seen from approximately image 2 through 9 of series 6 on the post-contrast evaluation and image 24 through 32 of series 5.  There is degenerative joint disease and slight joint space loss.  There is no fracture or dislocation.  There is slight edema in the proximal tibia at the midline, which may be secondary to chronic changes and degenerative changes.  There is no significant joint effusion.  There is loss of the posterior patellar cartilage.  There is no Baker’s cyst.  There is no other significant abnormal enhancement.  The menisci show degenerative changes and there are changes consistent with degenerative meniscal tear in the posterior horn of the medial meniscus.  There is joint space loss in this region.  This extends to the inferior articular surface.  There are intrasubstance degenerative changes in the remainder of the menisci.

IMPRESSION:          Changes consistent with osteochondromas as discussed above and the most prominent one is noted in the tibia, but there is also one at the distal femur region.  There is no significant abnormal enhancement.  The visualized portion of the lesion in the femur shows no significant abnormality or cartilaginous cap region.  In the proximal tibia, there is slight irregularity and would recommend clinical correlation or follow-up to confirm stability and correlation with any prior studies.  I understand the patient has had prior surgery.  There is degenerative joint disease and a meniscal tear in the posterior horn of the medial meniscus.  Please see comments above.

MRI EXAMINATION OF THE RIGHT KNEE WITHOUT CONTRAST

FINDINGS:               Axial, sagittal, and coronal images were obtained on the 1.5 Tesla GE MRI.  The patella appears in good position and the posterior cartilage shows no gross abnormality.  There is no significant Baker’s cyst, but there are some prominent veins noted in the posterior aspect, which may represent varicose veins and would recommend clinical correlation or further evaluation as needed.  There is no fracture, dislocation, or contusion to the bony structures.  The anterior and posterior cruciate ligament, medial and lateral collateral ligament, and quadriceps patellar tendon appear intact.  There are changes consistent with a subtle tear in the posterior horn of the medial meniscus and anterior horn of the lateral meniscus.  The age of these changes is indeterminate and would recommend clinical correlation or further evaluation as clinically necessary.
